Blankets Donated by Knit-a-Thoners Print out this page and cut the tickets apart. (You can also download a printable PDF version here. Fill out one ticket for each blanket you are sending, and enclose the tickets in your package. Please fill out at least your name and email. Your address is optional -- if you win a prize we can email you for your mailing address, but it's easier for us if you enter it on the slip. If you don't have a printer, you can just write the information requested on slips of paper and enclose those with your blankets -- make sure to send one slip of paper for each blanket you send.
